- Obtain a replacement card issued at a campus ID Office
- A charge of $10 may be assessed for the replacement of a lost or stolen card
- Pay this fee at a campus Business Office and take the receipt to an ID Office
- If you have experienced a theft that includes your ID card, a free replacement may be provided if you provide a copy of an official police report as documentation. (Official report from a city, state or federal police department)
