Request for Evaluation
Previous Educational Experiences

Important: The Central Records Office requires that the following criteria are met before processing an evaluation request:
1) The student must be accepted for admission to TCC;
2) The student must be placed in a curriculum; and,
3) The student must be term-activated for the current semester/term.

RECORDS TO BE EVALUATED. Below, please specify the name(s) of the school or agency from which we should be receiving OFFICIAL records that document previous educational experiences that you have had. To be considered official by us, the official record must be sent directly from the college/agency to the Central Records Office at the following address:

The record must also be current (i.e., not older than one year from the actual issuance date).
