ACADEMIC ALERT PROGRAM-- For Faculty

OVERVIEW

 Are there students in your classes who:

  • Are at risk of failing or being withdrawn from class?
  • Have low quiz and/or test scores?
  • Could use study skills assistance?
  • Have difficulty with reading, writing, or English as a Second Language?
  • Have personal issues that interfere with class attendance?
  • Are not utilizing their recommended accommodations?

The Academic Alert Program is designed to:

  • Provide faculty with a method early (prior to midterm) in the semester to identify at risk students
  • Address difficulties that interfere with student success
  • Refer students to appropriate college resources
  • Assist students in becoming academically successful
  • Form a collaboration between faculty and counselors to provide an effective system of early intervention to improve student retention and success.
  • Provide all faculty members with the opportunity to participate in the early alert program. It is each individual faculty member's choice whether or not to participate. Faculty submit the initial referral form to the campus contact. Upon receipt of the referral form, the designee contacts the students in regards to the referral. At that point, the designee work with students to determine the services and interventions that will be provided.

Faculty may also submit a hard copy or electronic copy to their designated campus office.

Please be sure to let your students know that you have identified them for the Academic Alert Program and to expect an e-mail correspondence and letter about the program.

Student Contact: The designee will contact the referred students by e-mail and letter.  Students are encouraged to contact the designee within two weeks from the date of referral. 

  • 1 week after the initial referral, students who have not responded will receive either a phone call or e-mail to offer another opportunity to contact the office for assistance.
  • 2 weeks after the initial referral, the designee sends follow up notes for those students who came in OR a “no-meet” form indicating the students who did not respond

During the initial appointment:

  • student signs the follow up form providing permission to inform referring instructor of topics addressed during meeting
  • student completes an initial evaluation form and study skills checklist to determine their needs
  • if applicable, an academic intervention form may be completed
  • handouts regarding student success may be given and techniques discussed regarding main areas of concern
  • student may be referred to Math Lab, Tutoring Center, Writing Center, Women’s Center, Counseling, etc.

During the semester: Students are able to continue coming in for subsequent sessions if they desire or if the designee feels it may be beneficial to the student.  

Follow up: Designee will send end of the semester follow up information to each instructor who participated in the Academic Alert program. The follow up may include the names of all referred students, whether or not they had contact with the designee, and their final grade in the course.

Deadline: Once the withdrawal deadline has passed, the designee will submit follow-up information to the faculty member regarding the referred students.
