FINANCIAL AID FORMS FOR THE 2012-13 AWARD YEAR
Below you will find a description of forms commonly used in the financial aid application process. Any necessary forms must be submitted to your campus financial aid office.
Verification Worksheet
If you were selected for verification, you received an official request from TCC for certain personal and financial documents. Included with this request was the Verification Worksheet. Please submit this completed form with all supporting documentation to your campus financial aid office. Please remember that you only need to do this if you were selected for verification.

Special Circumstance Review
If you have experienced a change in your financial circumstance, that you were unable to address when completing the Free Application for Federal Student Aid (FAFSA), you may complete this form and submit it to your campus financial aid office for review. This review is available to families who have experienced a significant decrease in household income since the previous tax filing year.

Dependency Status Appeal
If you are unable to provide your parent(s) information on the Free Application for Federal Student Aid (FAFSA) due to an unusual family circumstance, you may complete this form and submit it to your campus financial aid office for review. This review is available to students who have a demonstrated circumstance that prohibits them from providing parent information, but not because of a parent’s unwillingness or inability to contribute to educational expenses.

Income Certification
If you stated on the Free Application for Federal Student Aid (FAFSA) that you have a child or dependent for whom you provide at least half of the financial support, but your income is unusually low, you may be directed to complete this form and submit it to your campus financial aid office for review.
Permit to Disclose Personally Identifiable Information
If you wish to authorize the release of information regarding your financial aid eligibility to a third party, including a parent, spouse, or service agency, this form must be completed. This is a requirement of the Family and Educational Rights and Privacy Act (FERPA) of 1974, as amended. Please submit this form to the campus financial aid office.
Overlapping Student Loan Report 2012-13
Students who received Federal student loans at another institution may be required to complete this report before they may receive additional student loans at TCC.
