Student Delete Form

Please note: Only instructors are authorized to complete this form. This form is designed to document student deletes. A “student delete” refers to a student who has never attended your course. Such students should be removed from the course roster before the drop deadline (Last day to drop a course and receive a tuition refund). Please consult the academic calendar to verify the deadline dates for each session. By completing this form, I acknowledge the following: • If a student is marked as having never attended, the student will be deleted from the course, the course will be removed from their academic record, and a tuition refund will be issued.
