Admissions and Enrollment forms

Name of form	Description
Academic Renewal Petition	Students who return to the college after a separation of five years or more (i.e., 60 months or greater) may petition for academic renewal. If students meet eligibility requirements for academic renewal, D and F grades earned prior to re-enrollment may not be calculated into the cumulative and curricula GPAs, subject to specific conditions as outlined in the policy.
Application for Admission or Readmission after Academic Suspension or Dismissal form	Admission request for students who have been suspended or dismissed at another institution due to academic reasons.
Application for Admission or Readmission after Suspension or Dismissal for Student Conduct form	Admission request for students who have been suspended or dismissed at another institution due to disciplinary reasons.
Application for Credit by Local Examination or Experiential Learning (Portfolio) Credit form	To receive credit for college-level knowledge gained outside of the classroom, you can document what you’ve learned in a portfolio that can be assessed for academic credit.
Campus of Record (Intercampus Transfer form)	Students should submit this form to request a transfer to a different TCC campus. Students only need to request a transfer if they are a part of one or more of the following: Federal Work Study, Health Professions, Trucking, Veterans Affairs, Scholarship recipient, club/organization.
Enrollment Request form	Requesting enrollment in courses.
FERPA Consent to Release Educational Records	Authorization to release your information to a third party.
FERPA Prohibit the Release of Directory Information	Request for your directory information to not be released to a third party.
Parent or Legal Guardian Signature form	Submit this form if you are under the age of 18.
Petition for Incomplete Grade	Use this form to request an incomplete grade if you are unable to complete a course within the normal course time.
Petition for Late Withdrawal	Submit when seeking withdrawal from courses past the withdrawal deadline.
Petition to Audit a Class	Submit a petition to audit a class.
Student Data Change form	Submit to change your student information with the college.
Student Tuition Assistance for Senior Citizens and Guidelines for Eligibility	Request for tuition assistance for senior citizens.
Term Change Request form	Submit this form if you have applied to the College and would like to change the entry term indicated on your application.
Verification and Certification Release of Information	The Family Education Rights and Privacy Act of 1974 (FERPA), amended, states that a student must authorize in writing the release of her or his educational records to a third party.

Advising Forms

Name of form	Description
Authorization to Enroll in a Course More Than Twice	Submit if you are requesting to retake a course more than twice.
Course Overload Form	Submit this form if you are interested in enrolling in more than 18 credit hours in a single semester.
Curriculum Change Request Form	Complete this form to request that your curriculum be changed in the Student Information System (SIS).
Prerequisite Waiver Form	Complete this form if you are registering for a class at TCC and have been asked to demonstrate that you have completed the prerequisite at a previous college or university, and your official transcripts have not yet been evaluated.
Visiting Student form	Request to review prerequisites and enrollment assistance for students visiting the College from their home institution.

Domicile/Residency (in-state eligibility) forms

We strongly recommend that you watch this helpful video before completing any domicile forms. All documents should be submitted to the secure Registrar portal.

Name of form	Description
Domicile Exception Application	This form is for active duty military members, their spouses/dependents, and veteran and spouses/dependents who are using the GI Bill and reside in Virginia, but may not be domiciled in Virginia for purposes of in-state tuition.
Domiciliary Reclassification Application	Use this form if you are currently classified as out-of-state for tuition purposes, but feel you should be considered in-state.
Tuition Equity Provision (High School Completers)	Use this form if you are a Virginia high school completer requesting in-state tuition via a domicile exception.
Domiciliary Appeal for In-State Tuition Rates	Use this form if you would like to appeal the denial of your reclassification attempt.

Bookstore Authorization form	Complete to use financial aid to pay for your books.	Can only be completed online through your Student Information System (SIS). Login to MyTCC, View Action Items and Messages. The form is on your To-Do List.
Change in Marital Status form	Use this form if you (or your parents, if a dependent student) incorrectly reported their marital status on the FAFSA.	In-person
Dependency Status Appeal form	If you are unable to provide your parents’ information on the Free Application for Federal Student Aid (FAFSA) due to exceptional family circumstances, you may complete this form and submit it to your campus financial aid office for review. Your parents’ unwillingness and/or inability to financially support you or living on your own are not, by themselves, considered to be exceptional circumstances. Disability Certification form	Use this form to certify your eligibility for federal direct loans due to having prior loans discharged due to a total and permanent disability.	In-person Financial Aid Document Submission Portal
Financial Aid Cancellation form	Use this form if you want to cancel financial aid that you have been awarded.	In-person Financial Aid Document Submission Portal
Financial Aid Reinstatement form	Use this form if you previously cancelled your financial aid and would like the financial aid to be reinstated.	In-person Financial Aid Document Submission Portal
Income Certification form	If you stated on the Free Application for Federal Student Aid (FAFSA) that you have a child or dependent for whom you provide at least half of the financial support, but your income is unusually low, you may be directed to complete this form and submit it to your campus financial aid office for review. Independent Student Household Verification form	Use this form to verify the number of people in your household.	In-person Financial Aid Document Submission Portal

Overlapping Student Loan Report	Students who receive federal student loans at another institution may be required to complete this form before receiving additional student loans at TCC.	In-person Financial Aid Document Submission Portal
Private Scholarship Donor form	This form is for use by individuals and organizations that are providing scholarship funds for a student.	In-person

SAP Appeal form	Use this form if your financial aid has been suspended for not meeting the Standards of Satisfactory Academic Progress.	In-person
Special Circumstance Request form	If you have experienced a change in your financial circumstance that you were unable to address when completing the Free Application for Federal Student Aid (FAFSA), you may complete this form and submit it to your campus financial aid office for review.	In-person

Military Connected Forms

Name of form	Description
CMVE MyCAA Education Plan Request form	Use this form to request a MyCAA education and training plan.
CMVE TA Hold Request form	This form is used to request a hold be placed on your classes while you wait for your military tuition assistance (TA) voucher.
CMVE VA ECR form	Enrollment Certification Request (ECR). Students must submit this form every term they intend to use VA or VMSDEP benefits.
Student Veterans of America Interest Form	Submit this to join the Student Veterans of America.
Reserve Officers Training Corps and Workforce Programs	Please fill out this form if you are interested in one of our workforce development programs.

Registrar/Graduation forms

Name of form	Description
Graduate Name Card	This form provides your name card for Commencement, which will be used to announce your name and program during the ceremony.
Request Official Transcripts	This form is used when you request your official transcript from a campus. The fee is $9.75.
Request Transfer Evaluation form	Please submit this form to request any transcript evaluation (Ex. Colleges, JSTs, Credit for Prior Learning)
Requesting a Replacement Diploma	You may request a replacement diploma (degree or certificate), provided that you do not owe any outstanding debts to the college.
