Admissions and Enrollment forms

Academic Renewal Petition | Students who return to the college after a separation of five years or more (i.e., 60 months or greater) may petition for academic renewal. If students meet eligibility requirements for academic renewal, D and F grades earned prior to re-enrollment may not be calculated into the cumulative and curricula GPAs, subject to specific conditions as outlined in the policy. |
Application for Admission or Readmission after Academic Suspension or Dismissal form | Admission request for students who have been suspended or dismissed at another institution due to academic reasons. |
Application for Admission or Readmission after Suspension or Dismissal for Student Conduct form | Admission request for students who have been suspended or dismissed at another institution due to disciplinary reasons. |
Application for Credit by Local Examination or Experiential Learning (Portfolio) Credit form | To receive credit for college-level knowledge gained outside of the classroom, you can document what you’ve learned in a portfolio that can be assessed for academic credit. |
Campus of Record (Intercampus Transfer form) | Students should submit this form to request a transfer to a different TCC campus. Students only need to request a transfer if they are a part of one or more of the following: Federal Work Study, Health Professions, Trucking, Veterans Affairs, Scholarship recipient, club/organization. |
Enrollment Request form | Requesting enrollment in courses. |
FERPA Consent to Release Educational Records | Authorization to release your information to a third party. |
FERPA Prohibit the Release of Directory Information | Request for your directory information to not be released to a third party. |
Parent or Legal Guardian Signature form | Submit this form if you are under the age of 18. |
Petition for Incomplete Grade | Use this form to request an incomplete grade if you are unable to complete a course within the normal course time. |
Petition for Late Withdrawal | Submit when seeking withdrawal from courses past the withdrawal deadline. |
Petition to Audit a Class | Submit a petition to audit a class. |
Student Data Change form | Submit to change your student information with the college. |
Student Tuition Assistance for Senior Citizens and Guidelines for Eligibility | Request for tuition assistance for senior citizens. |
Term Change Request form | Submit this form if you have applied to the College and would like to change the entry term indicated on your application. |
Verification and Certification Release of Information | The Family Education Rights and Privacy Act of 1974 (FERPA), amended, states that a student must authorize in writing the release of her or his educational records to a third party. |

Course Overload Form | Submit this form if you are interested in enrolling in more than 18 credit hours in a single semester. |
Curriculum Change Request Form | Complete this form to request that your curriculum be changed in the Student Information System (SIS). |
Prerequisite Waiver Form | Complete this form if you are registering for a class at TCC and have been asked to demonstrate that you have completed the prerequisite at a previous college or university, and your official transcripts have not yet been evaluated. |
Visiting Student form | Request to review prerequisites and enrollment assistance for students visiting the College from their home institution. |
Domicile/Residency (in-state eligibility) forms
We strongly recommend that you watch this helpful video before completing any domicile forms. All documents should be submitted to the secure Registrar portal.

Domicile Exception Application | This form is for active duty military members, their spouses/dependents, and veteran and spouses/dependents who are using the GI Bill and reside in Virginia, but may not be domiciled in Virginia for purposes of in-state tuition. |
Domiciliary Reclassification Application | Use this form if you are currently classified as out-of-state for tuition purposes, but feel you should be considered in-state. |
Tuition Equity Provision (High School Completers) | Use this form if you are a Virginia high school completer requesting in-state tuition via a domicile exception. |
Domiciliary Appeal for In-State Tuition Rates | Use this form if you would like to appeal the denial of your reclassification attempt. |

Dependency Status Appeal form | If you are unable to provide your parents’ information on the Free Application for Federal Student Aid (FAFSA) due to exceptional family circumstances, you may complete this form and submit it to your campus financial aid office for review. Request Transfer Evaluation form | Please submit this form to request any transcript evaluation (Ex. Colleges, JSTs, Credit for Prior Learning) |
Requesting a Replacement Diploma | You may request a replacement diploma (degree or certificate), provided that you do not owe any outstanding debts to the college. |
